The purpose of this pack is to:

Please complete the following documents and return as instructed on your letter of offer as soon as possible:

  1. New Employee Personal Information Pack Printable PDF (363 KB), Fillable DOC (68 KB)
  2. Certified copies of the following documents:
    1. Proof of Date of Birth - Birth certificate or current passport
    2. Proof of Change of Name (if applicable) – Marriage certificate, change of name, deed poll or decree nisi
    3. Proof of Australian Citizenship or Permanent Residency - Citizenship certificate, current passport etc
    4. Proof of qualifications or registrations (if applicable)
    5. Identity as required by the Australian Criminal Intelligence Commission, please go to Nationally Coordinated Criminal History Check for further information.
  3. Medical Assessment Form (not for casual or temporary employment and only if requested, usually for permanent positions only) Health Assessment Form (84 KB).
